These forms are for use after

  • You have registered and entered the NAPDC Open division,
  • You have registered for the Presentational Showcase 
  • You have been accepted into the NAPDC Pro division.

PLEASE READ ALL DIRECTIONS IN EACH SECTION BELOW AND ON THE ACTUAL FORMS CAREFULLY TO AVOID MISTAKES

Competitors for open and pro divisions:

  • There are SIX (seven for doubles division) steps below which you must comply with – some now and some have future deadlines.
  • Failure to follow these steps correcty could result in penalties up to and including point deductions or disqualification.
  • Participants in the doubles division must each sign their own contract, waiver and stage name. Only one form is submitted for music, intro, props and pole settings.

Presentational Performers:

  • There are FIVE steps below which you must comply with.
  • All of the following forms EXCEPT the contract and the pole settings are required by performers in the presentationals even though they are not officially competing.
  • Some are due immediately and some later.

Note: Pole settings for presentationals will all be stage right on spin and stage left on static.

Anyone submitting to more than one division in either the pro, the open or both:

  • There is a different contract for pro vs open divisions. If you are in both, you must complete all three pages of each one.
  • If you are in two different open divisions, you may sign one open division contract but print both/all divisions under the division name on page three
  • If you are in more than one event you only need to sign one waiver
  • If you are in more than one event you must submit your stage name and intro separately for each event using the online forms.

 

1. Complete your contract –

  • Within 72 hours of registering for your division, download your contract here, hand sign it, and fax (708-754-9001) or scan (DO NOT TAKE PHOTOS OF YOUR CONTRACT) and email it back to us at info@MidwestPoleDancing.com
  • EACH performer in doubles must sign their own contract
  • Please indicate your division in the subject line or in the body of the email
  • If you are participating in more than one open division, you must submit an additional copy of page 3 of your contract.
  • If you are competing in a pro division as well as an open division you must submit a separate contract for each as there are differences in these contracts

2. Complete your waiver –

  • Within 72 hours of registering for your division, download your waiver here, hand sign it, and fax or scan/email (DO NOT SEND PHOTOGRAPHS OF YOUR WAIVER) and send it along with your contract by email or fax to info@midwestpoledancing.com or fax: 708-754-9001
  • EACH performer in doubles or troupes must sign their own waiver.

3. Reserve your music -

*Note…changes to your choice of reserved music may be submitted up until the deadline by sending a new form but all deadlines are firm.

  • You can reserve your choice of song at any time without sending the song file but no later than 11:59pm CST on June 6th
  • The latest date you may submit your song file is June 7th at 11:59pm CST
  • Maximum length for International Elite and Midwest Elite is 4:30. All other divisions 4:00
  • Your song file in MP3 or M4a format must be emailed to us at Info@MidwestPoleDancing.com
    using the subject line "Music (your division)" no later than 11:59pm CST on June 7th
  • Your song file MUST be saved/titled as: "Your Stage Name_Song Title.mp3" etc

4. Submit your performance introduction – Click here (all divisions use the same link)

*Note…changes to your intro may be submitted up until June 21st by sending a new form

  • Due within 72 hours of registration for the open divisions or after acceptance for the pro divisions.
  • Must be written in third person (do not use words such as "I", "Me", etc.)
  • May not be longer than 50 words
  • Only one intro per performance….doubles and troupes may have 75 words.

5. Confirm your name/stage name – Click here (all divisions use the same link)

*Note…changes to your name may be submitted up until June 21st  by sending a new form

  • Due within 72 hours of registration for the open divisions or after acceptance for the pro divisions.
  • Submit this if you would like a stage name listed next to your name on the website and in the event program
  • Submit this form if you wish to have a different name approved for use instead of your real/legal name
  • Submit this form if  you would like an abbreviated version of your real/legal name
  • This is the name that will be used in print, on our website, on your video from your performance, etc.
  • EVERYONE must submit this form to confirm how you would like your name to appear in print even if you are using your real/legal name.
  • EACH partner in the doubles division must submit this form for their own name
  • DBA/Assumed names are only good in the state in which they are issued. In order to use your DBA/Assumed name at the performance it must be registered in Illinois

***Important: You may not use a fictitional name. You must use your name, your first or middle name, your last name or initial or some combination of your real name as it appears on your drivers license/passport unless you have proof of a legally registered dba/assumed name. We will be requiring copies of your registration papers for your dba/assumed name or your drivers license/passport. If you have proof of regular use of a nick name that is a "real" name (not thaterical like butterfly or golden girl, etc) we will consider your request with proof of regular use. However you  may not erroneously choose a real name that does not belong to you.

6. Submit prop approval – Click here (all divisions use the same link)

*Note…changes to your notice of prop use may be submitted up until the deadline by sending a new form but all deadlines are firm. If you choose to NOT use a prop you have submitted you do NOT have to submit a new form.

  • Due June 21st
  • If you are intending to use a prop you must have approval
  • Examples (but not limited to) props which will not be approved: Pyro, devices which hang from the pole, glass, glitter, etc.
  • All props must be approved!
  • Everyone must submit this form to confirm if you are or are not using a prop!

7. Doubles division only – Submit your pole setting

*Note…changes to your pole settings may be submitted up until the deadline by sending a new form but all deadlines are firm.

  • Doubles division may choose one pole on static and one on spin (stage R MUST be on static with this option) or both on spin or both on static.
  • You must submit this information no later than June 21st
